Referrals:
|
Most insurance carriers require a referral for an office visit , test or procedure. Please check with your Primary Care Physician's office, as you are responsible for bringing the referral form. Often the Primary Care Physician's office will fax the referral to us beforehand, which is fine as long as it reaches our office at least two days before your appointment.

Insurances:

We accept Medicare, Medi-Cal, and HMO coverage through
Brown&Toland (BTMG). We do not participate in most PPO programs, thus payment is required at the time of service. An insurance claim form will be provided to you which you submit to your insurance Co.
